Neuromodulator Results: When You'll See Them and How Long They Last

Clinical content from Jeffrey Vogel, MD, MPH – Chief Medical Officer, Boundless

The most common source of confusion for men starting neuromodulators is timing. When will I see something? How long will it hold? What should I actually be watching for? Here is a clear timeline across all three products offered at Boundless.

Onset: when the result begins

Dysport is the fastest. Most men notice the treated muscles beginning to relax within two to three days of injection, making it the right choice when there is a specific deadline to plan around.

Botox typically begins between days three and five. Full effect for Botox settles in by day seven to ten.

Daxxify onset begins between days two and five, with full effect established by day seven to fourteen. Daxxify's peptide stabilizer enhances nerve terminal binding, which slightly slows the initial absorption compared to Dysport, but extends the duration meaningfully once binding occurs.

The correct time to assess any result is at two weeks. Evaluating earlier than that produces misleading impressions.

Peak results and duration

Botox and Dysport: peak results are visible from approximately weeks one to two onward. Duration for most men is three to four months. Men with stronger baseline muscle activity may find results wearing off closer to three months. Men with less dominant muscle activity, or those well into a long-term protocol where treated muscles have weakened from sustained relaxation, may extend closer to four to five months.

Daxxify: the SAKURA clinical trials documented a median duration of 24 weeks – approximately six months – with approximately half of participants maintaining meaningful improvement at that point. A significant proportion maintained results at nine months. For most men, the practical experience is results that hold solidly through month four to five, then begin softening gradually rather than dropping off sharply. What affects how long your results hold

Baseline muscle strength is the biggest variable. Men with very active, dominant glabellar or forehead muscles may find results wear off somewhat faster than the clinical average. This is addressed through dose calibration across sessions – your specialist builds a more accurate picture of your response pattern over successive appointments.

Consistency of treatment over time genuinely changes the baseline. Men who maintain regular treatment intervals find that the muscles being treated gradually reduce in activity due to sustained relaxation. Over years of consistent protocol, many men find that their results last longer and the dose required to maintain them decreases. This cumulative effect is one of the strongest long-term arguments for starting and staying consistent.

Individual metabolism and biological variation affect how quickly the product is cleared, in ways that are not fully predictable before the first treatment cycle.

When to schedule the next appointment

The right time to retreat is just before the treated muscles have fully returned to baseline – not after. Scheduling when you first notice early return of muscle movement, typically around month four to five for Daxxify and month two to three for Botox and Dysport, maintains more consistent results and gives the skin less time to re-crease between sessions.
