Neuromodulators as Skin Strategy: The Case for Starting Before You Think You Need To

The best time to start neuromodulators is before the lines become permanent. Here's the case for starting early – and the skin biology behind it.

Neuromodulators as Skin Strategy: The Case for Starting Before You Think You Need To

Clinical content from Jeffrey Vogel, MD, MPH – Chief Medical Officer, Boundless
"The men who start earlier have less to correct and more to protect. The preventive case for neuromodulators is genuinely stronger than the reactive one, and many men have no idea." – Boundless

The difference between dynamic lines and static lines

Dynamic lines form when a muscle contracts. They appear when you frown, squint, or raise your brows, and they disappear when your face relaxes. In your 20s and early 30s, these are dynamic lines – fully reversible, present only in expression.

Static lines are what those same dynamic lines become after years of repetitive creasing. The skin, repeatedly folded thousands of times per day along the same pathways, loses collagen density in those areas faster than surrounding skin. Over time, the line that only appeared during expression begins to appear at rest as well. It is no longer a muscle-driven line – it is a structural change in the dermis.

This distinction matters because neuromodulators address dynamic lines directly and static lines only partially. A man whose forehead lines are visible at rest has already crossed from the dynamic phase into the static phase in that area. Treatment can still meaningfully reduce the appearance of those lines – but it cannot undo the structural change that made them permanent. That structural work requires regenerative treatment. [Refer to this post for more information.]

What starting early actually prevents

When a neuromodulator prevents the glabellar muscle from contracting – across dozens of daily expressions, 365 days a year – it prevents thousands of repeated mechanical creases from being reinforced in the skin. Over years, this is not a cosmetic effect. It is a reduction in the rate at which dynamic lines become static ones.

The men who have maintained consistent neuromodulator treatment since their late 20s or early 30s genuinely show it – not because they look "done," but because the lines that would have etched into their skin from a decade of unchecked expression movement simply are not as deep as they would otherwise be.

The right age to start

There is no single right age. The right time is when your dynamic lines are visible under normal expression and beginning to show any trace at rest. For many men, that is the early 30s for glabellar lines, and the mid to late 30s for forehead lines.

Starting before the lines become static does not mean starting unnecessarily early. It means starting before the treatment is working primarily to correct structural damage rather than to prevent it.

We feel that framing neuromodulators as a reactive treatment – something you do after the lines become a problem – misses most of their value. A preventive protocol is more effective, less costly over time, and requires less intervention to maintain good results. [Check out this post for Botox as a starting point, and view this post for Daxxify as a long-term protocol option.]

What a long-term protocol actually looks like

Two Daxxify appointments per year is the most efficient long-term approach for most men. The treated muscles gradually weaken from sustained relaxation across years of consistent protocol. Results last longer, doses decrease, and the cumulative skin quality benefit of fewer mechanical creases compounds over time.

Men who maintain this consistently through their 40s and 50s do not need more intervention as they age – in many cases, they need less.
