What to Expect at Your First Neuromodulator Session at Boundless

Clinical content from Jeffrey Vogel, MD, MPH – Chief Medical Officer, Boundless

Most men who walk into their first neuromodulator appointment do not know exactly what will happen once they are there. Here is the full sequence – before, during, and after – so you know what to expect.

Before you arrive

No significant preparation is required for a neuromodulator session. A few common-sense steps in the days prior help reduce the chance of bruising and ensure the session produces the cleanest possible result.

Pause NSAIDs – ibuprofen, aspirin, naproxen – and blood-thinning supplements including fish oil, vitamin E, and garlic for five to seven days before your appointment. These thin the blood and increase bruising risk at injection sites, which resolves on its own but can be avoided with advance planning. Arrive well hydrated. Avoid alcohol for 24 hours before the appointment.

During the session

A standard neuromodulator appointment at Boundless runs 20 to 30 minutes including the assessment. Here is the sequence.

The session begins with a facial assessment. Your specialist evaluates your facial muscle activity patterns, how your specific muscles move under concentration and at rest, the distribution and depth of existing lines, and your aesthetic goals. For returning patients, this includes a review of how the previous treatment performed. This is the most important phase of the appointment – the quality of your result is largely determined by what is observed here.

Treatment planning follows. Your specialist maps the specific injection points, doses, and muscle targets for your face. This is not a standardized template. Placement and dosing are specific to your anatomy and what you want to achieve.

The treatment area is cleaned and injection points are marked. Topical numbing cream is available if you prefer it, though most men find it unnecessary – the needles used are extremely fine-gauge and each injection takes only a few seconds. Most men describe the sensation as a series of brief, mild pinches.

Injection takes five to ten minutes for a standard treatment. A typical glabellar treatment involves five injection points. You will receive aftercare instructions and can leave immediately.

The 24 hours after

Stay upright for two to four hours after the appointment. Avoid lying face down during this window. Skip vigorous exercise, saunas, and significant heat exposure for 24 hours. Avoid rubbing or massaging the treated area.

Mild redness or small raised areas at the injection sites are normal and typically resolve within 20 to 30 minutes. Mild bruising is possible and resolves within a few days if it occurs.

The two weeks after

Onset begins within two to five days, depending on which product was used. Dysport tends to be the fastest. Daxxify and Botox typically begin between days two and five. Full effect is established by day seven to fourteen for all three.

Resist the urge to evaluate the result before day ten. Many men check too early and either overestimate (the first few days of partial onset look uneven) or underestimate (expecting full effect by day five). The two-week mark is the right time to assess the result and communicate with your specialist about any areas that would benefit from refinement. A small touch-up dose placed at the two-week follow-up is an option if any area is under-treated. [Check out this post for more information.]

Final Thoughts

A well-run neuromodulator session is efficient, low-discomfort, and requires no adjustment to your schedule beyond the 20 to 30 minutes you are there. The preparation is minimal, the session is quick, and recovery is immediate. [Check out this post for how often you will need to return.]
