What to Expect in the Weeks After an Exosome Session

Clinical content from Jeffrey Vogel, MD, MPH – Chief Medical Officer, Boundless

Men who come in for their first Exosomes + Microneedling session often leave with the same question: now what? Here is an honest timeline of what to expect, including the phase that most commonly causes men to doubt the protocol unnecessarily.

The first 24 hours

Immediately after treatment, the scalp may appear flushed and feel warm and tight. This is normal – it is the repair response the microneedling component was designed to trigger. Keep the scalp clean, avoid washing your hair, skip intense workouts and saunas, and avoid anything that causes significant sweating.

Days two through seven

Redness and sensitivity typically resolve within 24 to 48 hours. Resume gentle washing with a mild, fragrance-free shampoo. Hold off on minoxidil or other medicated scalp products for three to five days. NSAIDs should be avoided for approximately 72 hours post-treatment. The scalp will be more photosensitive than usual – use sun protection when outside.

Weeks one through eight: the shedding phase most men misread

The first visible sign that a protocol is working is typically a reduction in shedding – but some men experience the opposite initially. Temporary increased shedding in the first four to eight weeks is documented in the literature and indicates that the protocol is shifting follicles toward a more active state (Natarelli et al., Journal of Clinical Medicine, 2023). Hairs that were already in the resting phase shed ahead of new growth.

Dr. Vogel notes that this is the phase where most men want to quit – and the single most important thing is to push through it. The shedding resolves. [See this post for a broader discussion of why men quit too early.]

Months three through six: visible change

The more meaningful improvements typically emerge in this window. Hair in treated areas begins to feel fuller, coverage appears denser, and changes become visible in the mirror. This is when most men can see something worth pointing to.

Hair cycles are long. A follicle that was in the resting phase during your first session may only now be in active growth.

Final Thoughts

Recovery is minimal. Results are gradual and follow the follicle's timeline, not the calendar's. Monthly progress photographs – same lighting, same angle, scalp slightly damp – make gradual improvement visible in a way that daily observation cannot. [See this post for a full guide to tracking progress.]
